Explanation of Benefits

An explanation of benefits, or EOB, is a statement from your health insurance plan describing what costs it will cover for medical care or products you’ve received.

6. In-Network Provider

An in-network provider is a healthcare provider in contract with your insurance to provide services to plan members at a pre-negotiated rate.

7. Out-of-pocket max

The out of pocket max is the most you will pay in a given year for all covered services. This is a combined total of deductible, coinsurance and copayments made for any one individual. Once the out of pocket max has been met, all covered services are covered with no additional cost share.

8. Premium

A premium is the amount an individual or employer pays each month for your health insurance or plan. You will learn of the amount you are responsible for at open enrollment each year depending on the plan you choose, and typically see that amount withdrawn from one monthly paycheck, or split between two bi-weekly paychecks.

9. Prior authorization or Pre authorization

Prior authorization means getting approval before you can get access to medicine or services. This is not always necessary, but you should call your health insurance company using the number on the back of your ID care before any surgery, diagnostic, lab or other service you are going to receive to ensure your insurance provider has agreed to cover the service.

10. Referral

A referral is a written order from your primary care provider for you to see a specialist or get certain health care services. Not all health plans require a referral, but if your plan does, ask your PCP or clinic for an electronic referral before you visit a specialist. Without this referral, you’ll likely pay more for your care or it may not be covered. If you don’t know if you need a referral, sign in to your health plan account or call the number on your ID card to find out. Your ID card may even say "Referrals Required."

Now that you have a better understanding of these 10 common terms, you are one step closer to being the best healthcare consumer you can be!

Of these, blood pressure is one of the most critical because high blood pressure greatly increases your chance of heart attack or stroke, and often has no signs or symptoms. Starting at age 20, the American Heart Association (AHA) recommends having your blood pressure checked at least once per year at your annual physical exam. Although, your doctor may recommend more frequent screenings if your blood pressure is higher than normal.

Your “lipoprotein profile” or cholesterol as it’s more commonly known, should be tested every 4 to 6 years after age 20. Like blood pressure, your doctor may recommend more frequent screenings based on your risk factors.

For those who are overweight and have at least one additional cardiovascular risk factor, your healthcare provider may ask for a blood glucose or “blood sugar” test. High blood glucose puts you at a greater risk for developing prediabetes and type 2 diabetes. When uncontrolled, diabetes can lead to heart disease and stroke.

While it all may sound a little dismaying, having these screenings can have many potential benefits. When you know your risks, you can work with your doctor to make positive changes in your health before serious illness occurs. Moreover, conditions are easier to treat when caught early on. If some of your readings are less than optimal, lifestyle changes and/or medication can often be used to control conditions such as high blood pressure and high cholesterol.

Note: Before visiting the doctor for an annual exam, be sure your chosen physician is in-network according to your health plan. Sodium is a necessary mineral for our bodies that is involved in muscle function and sending nerve impulses. However, consuming too much sodium can put extra strain on your heart and blood vessels and lead to high blood pressure for some.

The AHA (American Heart Association) suggests eating no more than 2,300 milligrams (mg) of sodium per day and ideally limiting your daily intake to 1,500 mg per day for most adults. Often, premade, prepackaged foods contain high amounts of sodium. Read food labels, especially on premade breads, pizzas, soups, sandwiches, and cured meats, as these can be some of the biggest culprits when it comes to high sodium. Opt for the products with the lowest sodium content.

So much so, that what we know as today’s broad EAP scope stemmed from a single program. The original Alcoholics Anonymous (AA) was created to treat the negative effects alcoholism had on overall job performance. In the last 80+ years, EAPs have grown to encompass a multitude of services which tackle struggles faced both in the office and at home. EAPs aim to treat the effects these struggles have on employee well-being, productivity, and overall happiness.

Simply put, employee assistance programs offer resources for ALL employees to achieve a healthier work/life balance. While the utilization of EAPs has increased since 1940, these beneficial programs are still considered underutilized at a reported less than 10% (shrm.org). Why? Many employees are simply not educated on the depth and availability of EAPs.
