The Forsite Benefit Partners and Motion Connected Privacy Policy ("Privacy Policy") explains the privacy practices of FORSITE BENEFIT PARTNERS LLC AND MOTION CONNECTED LLC and our subsidiaries and affiliated companies (“Forsite Benefits”, "Motion Connected," "we," "us," "our") regarding the personal data and information ("information") we collect about you in connection with the resources and services available in our websites, mobile apps, Windows applications, emails, and accounts on social media platforms ("our services").

Consumer trust and confidence are important to us at Forsite Benefits and Motion Connected. We want to be transparent about how we collect, use, share and safeguard your personal information. This policy does not apply to our services that display or link to different privacy statements. Certain details of this policy may depend on which of our services you use.

Who we are

Forsite Benefits provides medical, dental, life and disability insurance benefits to individuals and corporations. Motion Connected provides wellness program services to organizations (for example, employers, charities, or cooperatives). Organizations subscribe to our services, and their affiliates (for example, their employees, volunteers, or members) register for and use those services. Motion Connected does not provide services to organizations that make affiliate participation mandatory or whose affiliates are unaware of their involvement in our services.

How we use personal information

We use the personal information we collect to: provide the services you choose to use, communicate with you, and understand how you use our services, so that we may improve them.

We may combine information you provide with information we collect automatically or receive from third parties. We may also anonymize and aggregate information so that it cannot be used to identify you or any other individual.

We are not in the business of selling, renting, or trading your personal information with others for marketing purposes.

Information organizations provide

Organizations provide us information necessary to establish a business relationship, such as their contact and billing information. Organizations may provide us information about their affiliates, such as name, date of birth, email address, and program status, that we need to provide the services to which they have subscribed.

Information you provide

When you choose to register with Motion Connected, you provide the following information to identify yourself and verify your eligibility to use our services:

• date of birth
• first name
• last name
• Login ID
• preferred security question
• security question answer
• time zone

If you require personal contact with Motion Connected (for example, for troubleshooting, to answer service questions, or to receive products), you may choose to provide:

If you use Motion Connected graph and charting services, you may choose to provide:

Timestamps, steps, activity time, distance, calories, weight, height,systolic/diastolic mmHg and pulse, HDL, LDL, and triglyceride glucose mg/dl and HbA1c.

If you participate in a Motion Connected scorecard, you may choose to provide: timestamps, description of activities, wellness-related data, supporting documentation, food journals, survey answers, observations and opinion.

Your organization may request you provide us with additional information to satisfy their program requirements, such as information about tobacco use or organized event participation. Contact your organization's wellness program administrator regarding additional optional and required information your program may require.

Information we collect automatically

When you visit our websites, we collect timestamp, page, Internet Protocol (IP) address, operating system identifiers, device identifiers, browser type and language, and the website you visited before visiting ours.

If you install our mobile apps, we collect operating system identifiers, device identifiers, Internet Protocol (IP) address, time-stamped logs of data exchanges associated with our mobile apps, and usage statistics associated with our mobile apps.

If you install our Windows applications, we collect operating system identifiers, .NET identifiers, device identifiers, Internet Protocol (IP) address, time-stamped logs of data exchanges associated with our Windows applications, and usage statistics associated with our mobile applications.

Information we receive from data partners

If you use an activity tracker supported by Motion Connected, you may link your Motion Connected account with your device manufacturer’s account. The link is established using anonymous information. Motion Connected then automatically acquires the following data: date, steps, distance, time, and active minutes.

Information we receive from third parties

We use Google Analytics to help us understand how you are interacting with our websites, such as referrer and how many times you visit a page. To learn more about Google Analytics practices,visit google.com/analytics

Forsite Benefits and Motion Connected uses Zoho to help us evaluate the effectiveness of our marketing communications, such as a marketing email or a marketing-based landing page on our website. To learn more about Zoho, visit zoho.com

Forsite Benefits and Motion Connected uses JotForm as a custom forms platform for the collection of first name, last name, and email and street address. We use this information to provide additional program services. To learn more about JotForm, visit jotform.com

Forsite Benefits and Motion Connected use Facebook, Twitter, LinkedIn and Blogger. You can connect with and follow us on these sites. If you connect with Motion Connected's accounts on third party social media, we may also collect information about your social networking accounts such as your name, user name, or comments related to Forsite Benefits or Motion Connected.

Motion Connected uses 3dcart to provide an eCommerce store. When you choose to use the eCommerce store, we provide 3dcart with your first name, last name, email address, and a group code that identifies your organization's purchasing program. We receive from 3dcart your shipping address, the products you purchased, price information, and payment method.

The third parties may also collect information. Forsite Benefits and Motion Connected's Privacy Policy does not cover the actions of these or any third parties, and Motion Connected cannot control how third parties may collect, use, or secure your personal information. For information about a third party's privacy practices, consult the privacy policy of that third party.

Information sharing

Motion Connected does not share your personal information except in limited circumstances described below. We may share anonymized and aggregated information publicly and with third parties for research, to improve our services, and to market our services.

Organizations may receive anonymized and aggregated information about affiliate participation in our services. We limit the personal information we share with organizations to the minimum necessary for their affiliates to meet the requirements of their wellness programs.

We may share your personal information with third parties to provide you with Forsite Benefits and Motion Connected services. We require our service providers to protect personal information they process on our behalf. Our service providers are not authorized by us to use or disclose your personal information except as necessary to perform services on our behalf, as specifically authorized by us or by you, or to comply with legal requirements.

Applicable law may require us to disclose your personal information if: it is reasonably necessary to comply with legal process (such as a court order, subpoena, or search warrant) or other legal requirements; disclosure would mitigate our liability in an actual or threatened lawsuit; it is necessary to protect the legal rights of Forsite Benefits and Motion Connected, our users, customers, business partners, or other interested parties; or it is necessary for the prevention or detection of crime (subject in each case to applicable law). For residents of the European Economic Area, Forsite Benefits and Motion Connected will disclose personal information only when permitted to do so under applicable European and EU Member States' national data protection laws and regulations.

You may choose to make certain details about yourself, such as your nickname and relative performance, visible to other participants of Motion Connected services. Any personal information you share can be read, collected, or used by other individuals in ways we are unable to control or predict, such as to send you unsolicited emails. We are not responsible for any personal information you choose to share in this manner.

Modifying and deleting your personal information

You can modify your personal information, and change your preferences on how we communicate with you, by logging into your Motion Connected account and accessing your profile. To remove your account and any personal information from our services, send us an email at assist@forsitebenefits.com or support@motionconnected.com. You will receive an email message confirming your account's removal. Forsite Benefits and Motion Connected will retain information that has been anonymized.

Email marketing choices

Our marketing emails tell you how to opt-out of receiving further marketing emails. You may also contact us at assist@forsitebenefits.com or support@motionconnected.com at any time to let us know that you no longer wish to receive marketing emails. If you opt out, we may still send you non-marketing emails, such as emails about your accounts and our business dealings with you.

Do not track

Currently there is no accepted standard for how to respond to Do Not Track signals, and we do not respond to such signals.
